If you are a company or school requesting graduation verification
for a former student, graduation request verification request forms
and releases can be sent to:

The request must include:
- the signature of the person subject to verification (former student, or parent/guardian if under 18),
- student's legal name,
- date of birth, and
- enrollment years at City Academy.
